Original United States Treasury Department "Internal Revenue Order Form for Opium or Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations Thereof" issued under the authority of the Harrison Narcotics Tax Act framework and related federal narcotics regulations. This historically significant government pharmaceutical control form is dated May 18, 1943, during the height of World War II. The document bears printed Treasury Department formatting with ornate engraved borders and references to the Act of Congress approved December 17, 1914. The form is designated "Series of 1936" and carries serial number A278832. Issued by "John E. Manning" to the "5TH N.J. CLASS 3&5 REG. 1694, MORRIS MAYER T/A, MAYER'S DRUGS" in Passaic, New Jersey. Handwritten entries document pharmaceutical narcotic purchases supplied to "D. Kaltman & Co." located at "176 Webster Ave., Jersey City, N.J." Listed substances include: Codeine Sulfate, Codeine Phosphate and Ethyl Morphine HCL. Signed at bottom by Morris Mayer with the reverse side left blank. Overall fine condition, with some toning and expected folds. Measures 8.125" x 10.75".

This document originates from the era of strict federal narcotics regulation established after the Harrison Narcotics Tax Act of 1914, one of the earliest major federal efforts to regulate and monitor the distribution of opiates and narcotic compounds in the United States. Under these laws, pharmacists, physicians, manufacturers, and distributors were required to maintain highly detailed transactional records for substances containing opium derivatives and narcotic preparations.

By 1943, during World War II, narcotic medicines such as codeine and morphine derivatives remained critically important in both civilian and military medical treatment. The federal government tightly controlled their movement through Treasury Department oversight and Internal Revenue registration systems.

We insure through Berkley Asset Protection with rates of $.70 per $100 of value, among the lowest insurance rates in the industry. Our shipping department cameras document every package, both outgoing and incoming, for maximum security. In addition, we compare our shipping and handling rates against those of other auction houses, to ensure that our charges are among the lowest in the trade.

Upon winning your item(s), you will receive an invoice with our in-house shipping and handling fees included. ***We will ship to the address as it appears on your invoice. If any changes to the shipping address need to be made, you must inform us immediately.***

International shipments: In order to comply with our insurance provider, all international shipments will be sent via Fed Ex and customs paperwork will show a value of $1.00. International buyers should contact our office directly with any questions regarding this policy.

Third-Party Shipping Option: If a third-party shipper is preferred, the buyer is responsible for contacting them directly to make shipping arrangements. For your convenience, we have provided some recommended shippers. For your protection, we will require a signed release from you, confirming your authorization for us to release your lots to your specified third-party. At that point, our responsibility and insurance coverage for your item(s) ceases. Items picked up by third-party shippers are required to pay Connecticut sales tax. Items requiring third-party shipping due to being oversized, fragile or bulky will be denoted in the item description.
