The President's Commission on the Assassination of President Kennedy, informally known as the Warren Commission, was established pursuant to President Lyndon B. Johnson's Executive Order 11130 issued on November 29, 1963. Gerald R. Ford, then a Michigan Congressman, was one of the commission's seven members. Fellow commissioners were drawn from the Supreme Court, Congress, and the C.I.A., as well as the ranks of longtime presidential advisers. Over the course of ten months, the commission conducted interviews and gathered intelligence, producing its nearly 1,000-page eponymous report in September 1964. The Warren Commission determined that Lee Harvey Oswald had alone killed JFK, and that Jack Ruby had alone killed Lee Harvey Oswald.

This limited edition issue of the Warren Commission report was released on the 40th anniversary of the publication of the original findings. Ford's fellow commissioners--Earl Warren, Hale Boggs, John Sherman Cooper, Allen Dulles, John J. McCloy, and Richard Russell, Jr.-- had all died between 1969-1989. Ford was indeed the Commission's sole survivor, a title he would hold until his own death two years later.
